firmware upgrades

For some reason the upgrade doesn't seem to be working to a device, I see the cwmp shows

The fc logs don't show anything except listening (I confirmed I can download the firmware url that's listed in the debug from the device copied and pasted into ie it downloads fine).
Even when I download via IE I don't see any notice in the fs log.

I see request, then response, then request, and then a response for the download response

As long as the download is in the device queue that 4 valuechange, 1 boot keeps reoccurring until I remove the task...

Any idea what I may be missing?
